Subject: Dispute of Fraudulent Account Creation

Dear [Sir/Madam],

I am writing to inform you that I am a victim of identity theft. Recently, I discovered that an account was opened in my name at your company without my authorization. I request that this account be closed immediately and that [Company Name] absolve me of all charges related to it. Additionally, I ask that you take all necessary steps to remove information about this account from my credit files.

I also request a copy of all information related to the application and account, including the unauthorized application details and a record of transactions or charges. Please provide the categories of identifying information used to open the account.

Enclosed are copies of my Identity Theft Report and my credit report, which highlight the fraudulent items related to your company. Unrelated information has been redacted for privacy. Also included is a copy of the FTC Notice to Furnishers of Information, which outlines your responsibilities under the Fair Credit Reporting Act (FCRA). Upon receiving a consumer’s written request with an Identity Theft Report, you are required to cease furnishing the information resulting from identity theft to any credit reporting agency (CRA).

The Notice also details your responsibilities under section 605B of the FCRA when you receive notice from a CRA that the information you provided may be the result of identity theft. These responsibilities include stopping the provision of inaccurate information to CRAs and ensuring the fraudulent debts are not sold or transferred for collection.

Please investigate this matter, close the account, absolve me of all charges, and fulfill the steps required under the FCRA. Kindly send me a letter explaining your findings and actions.
